• Current students, teachers, and personnel of Gordon Heights National High School
• Individuals with a valid school ID number issued by GHNHS
• Individuals who can provide accurate and truthful registration information

2.2 Account Registration

To use the Service, you must:

• Create an account using your real name and valid email address
• Provide your legitimate GHNHS school ID number
• Provide a valid Philippine mobile phone number for contact purposes
• Create a secure password meeting our security requirements
• Complete email verification through OTP (One-Time Password)
• Wait for admin approval before accessing full system features

2.3 Account Security

You are responsible for:

• Maintaining the confidentiality of your account credentials
• All activities that occur under your account
• Notifying us immediately of any unauthorized use of your account
• Ensuring your password is strong and not shared with others
• Logging out of your account at the end of each session

3.1 Purpose of the System

GHNHS MedTrack is designed to facilitate healthcare management within the school environment, including:

• Tracking of student and staff health records
• Management of medicine requests and inventory
• Scheduling and recording of health appraisals and checkups
• Communication between clinic staff and school community members
• Digital health profile management

3.2 Not a Substitute for Professional Medical Care

3.3 Medicine Request Services

When requesting medicines through the system:

• You must provide accurate information about symptoms and medical needs
• All medicine requests are subject to approval by authorized clinic staff
• Medicine distribution is limited to available stock in the school clinic
• Teachers and personnel may request medicines on behalf of students in emergency situations
• The system does not guarantee immediate availability or approval of medicine requests
• Prescription medications require proper documentation and may not be dispensed without authorization

3.4 Health Information Accuracy

You agree to:

• Provide accurate, current, and complete health information
• Update your health profile promptly when information changes
• Report any allergies, pre-existing conditions, or medications you are currently taking
• Not falsify medical information or health status
• Inform clinic staff of any adverse reactions or medical emergencies

• Share your account credentials with others or allow others to use your account
• Attempt to circumvent security measures or access restricted areas of the system
• Use automated tools (bots, scripts, scrapers) to access or interact with the system
• Upload or transmit viruses, malware, or any malicious code
• Harass, threaten, or abuse clinic staff or other users through the system
• Attempt to reverse engineer, decompile, or disassemble any part of the system
• Copy, reproduce, or distribute system content without permission
• Use the system in any way that could damage, disable, or impair its functionality

The GHNHS MedTrack system, including its design, features, functionality, source code, and all content (excluding user-provided health information), is owned by Gordon Heights National High School and is protected by Philippine and international intellectual property laws.

You retain ownership of your personal health information and data, but by using the system, you grant us a limited license to:

• Store, process, and display your information as necessary to provide services
• Use anonymized, aggregated data for statistical and research purposes
• Share information with authorized clinic staff for healthcare delivery

We strive to provide continuous access to the GHNHS MedTrack system, but we do not guarantee uninterrupted availability. The system may be unavailable due to:

• Scheduled maintenance and updates
• Technical issues or server outages
• Security incidents requiring temporary shutdown
• Force majeure events (natural disasters, power outages, etc.)

We reserve the right to modify, suspend, or discontinue any aspect of the Service at any time without prior notice. We are not liable for any interruption, suspension, or termination of the Service.

TO THE MAXIMUM EXTENT PERMITTED BY PHILIPPINE LAW, GHNHS, ITS ADMINISTRATORS, EMPLOYEES, CLINIC STAFF, AND AFFILIATES SHALL NOT BE LIABLE FOR:

• Any indirect, incidental, special, consequential, or punitive damages
• Loss of data, profits, or business opportunities
• Medical complications arising from information provided through the system
• Delays in receiving medical attention due to system availability
• Unauthorized access to or alteration of your data
• Actions or omissions of other users
• Any decisions made based on information from the system

This limitation applies even if we have been advised of the possibility of such damages.

You agree to indemnify, defend, and hold harmless GHNHS, its administrators, employees, clinic staff, and affiliates from any claims, damages, losses, liabilities, costs, and expenses (including reasonable attorney's fees) arising out of or related to:

• Your use or misuse of the GHNHS MedTrack system
• Your violation of these Terms and Conditions
• Your violation of any rights of another party
• Any false or misleading information you provide
• Any unauthorized access to the system using your account

11.1 Termination by You

You may request account termination at any time by contacting the school clinic administrator. Upon termination, you will lose access to all system features and services.

11.2 Termination by GHNHS

We reserve the right to suspend or terminate your account immediately, without prior notice, if:

• You violate these Terms and Conditions
• You are no longer affiliated with GHNHS (graduation, transfer, resignation)
• You provide false or misleading registration information
• Your account shows suspicious or fraudulent activity
• You engage in conduct harmful to other users or the system
• Required by law or legal process

11.3 Effects of Termination

Upon account termination:

• Your access to the system will be immediately revoked
• Your health records will be retained according to our data retention policy and applicable laws
• Any pending medicine requests may be cancelled
• You remain responsible for any obligations incurred before termination

Parents and guardians acknowledge that:

• They have reviewed these Terms and the Privacy Policy
• They consent to the collection and use of their child's health information as described
• They understand the system's limitations and that it does not replace professional medical care
• They are responsible for monitoring their child's use of the system
• They can request access to or deletion of their child's health information

These Terms shall be governed by and construed in accordance with the laws of the Republic of the Philippines, including but not limited to:

• Republic Act No. 10173 (Data Privacy Act of 2012)
• Republic Act No. 10175 (Cybercrime Prevention Act of 2012)
• Republic Act No. 8504 (Philippine AIDS Prevention and Control Act)
• Department of Health regulations and guidelines

Any disputes arising from these Terms or your use of the Service shall be resolved through amicable settlement. If resolution cannot be reached, disputes shall be submitted to the proper courts of the Philippines.
